50-59 Less than or equal to 3.5
60-69 Less than or equal to 4.5

normal or abnormal is valid only if this
patient has
never been treated for prostate cancer
and is not on any
medication that would change the PSA
value. Clinical
correlation is strongly recommended.
This PSA assay was performed using
a chemiluminescent immunoassay method.

FINDINGS:
The osseous structures are intact with
no fracture or focal
destruction. The joint spaces are
maintained. No erosions are
identified. Small spurs are seen at
the first carpometacarpal joints
bilaterally. No soft tissue
abnormality is identified.
IMPRESSION:
Minimal spurring at the first
carpometacarpal joints, otherwise
normal bilateral hands.
I have personally viewed this
procedure/exam and reviewed this report.

agreed upon for the patient by
consultation with the referring
physician Dr. Vanek. After positively
identifying the patient, the
patient was given an oral capsule
containing 23.2 milliCuries of
I-131. The patient was sent home with
printed post therapy
instructions to prevent radiation
contamination. No immediate
complications occurred.
IMPRESSION:
Successful administration of 23.2 mCi
I-131 for the treatment of
hyperthyroidism.
I have personally viewed this
procedure/exam and reviewed this report.

NM THYROID UPTAKE SINGLE DETERMINATION
(10/02/2008 2:08 PM PDT)
| Component |
Value |
Range |
| NM THYROID UPTAKE ONLY |
Clinical
history:
40-year-old male with history of eye
problems, symptoms of
hyperthyroidism, and other findings
raising concern for Graves'
disease. Patient has been off of
anti-thyroid medication
(methimazole) for approximately 6
days.
Comparison:
No comparable
Study:
I-131 uptake of 10/1 -- 10/2/08
Findings:
On 10/1 the patient received 8.3
microcuries of iodine-131 by capsule
on an empty stomach. He returns on
10/2 and a neck uptake is
calculated at 9.1%. Centering the
thyroid probe lower on the neck
gives an uptake of 17.5%.
Patient's neck is rather heavy set,
and gland size is estimated at 40
g. Findings were discussed with Dr.
Vaneck, and did a magnitude of
the his symptoms and disease, therapy
is considered worthwhile.
Therapy dose is calculated at
approximately 23 mCi.
IMPRESSION:
I-131 uptake is not elevated, but is
approximately 17.5 and a thyroid
gland placed low in the neck.
Potential for therapy is discussed
with Dr. Vanek and the patient.
END OF IMPRESSION:
I have personally viewed this
procedure/exam and reviewed this report.

following sequences:
Axial T1, T2, FLAIR, DWI and proton
density,
Sagittal FLAIR,
Sagittal T1 weighted,

with no evidence of hydrocephalus or
mass. The flow voids of the
cerebral vasculature appear normal.
There are no areas of
diffusion restriction is suggest
infarction. No intraparenchymal
hemorrhage or extra fluid collections
are seen. There is no mass
effect or shift of midline structures.
There is no abnormal
enhancement on the post gadolinium
studies. Mucosal thickening is
present within the left maxillary

609172 Estimated GFR > 60 mL/min/1.73 sq
m if African American
GFR is estimated using the MDRD equation
recommended by the
National
Kidney Disease Education Program.
Estimated GFR Interpretive Information:
<60 mL/min/1.73 sq m Chronic Kidney
Disease
<15 mL/mon/1.73 sq m Kidney Failure
Estimated GFR greater than 60mL/min/1.73
is of limited clinical
Value.
The MDRD equation is not valid in the
following situations:
- Patients under 18 years of age
- Severe malnutrition or obesity
- Vegetarian diet
- Rapidly changing kidney function

DIPYRIDAMOLE SESTAMIBI STUDY
COMPARISON STUDIES: None
HISTORY: Chest pain.
TECHNIQUE: The stress portion of the
study is first performed
under the supervision of nursing. EKG,
BP, and other pertinent
details are recorded in the chart. The
patient is infused with
59.6 mg dipyridamole; 32 mCi sestamibi
is injected two minutes
after the completion of the
dipyridamole infusion. Gated SPECT is
acquired one hour after the conclusion
of the infusion.
The rest portion was performed the
following day, the patient is
injected with 10.2 mCi Tc 99m
sestamibi. Gated SPECT imaging is
acquired one hour after injection.
SPECT and gated images were evaluated.
FINDINGS: A small defect at 11 oclock
is thought to be RV
insertion. Distribution of activity on
stress images is otherwise
normal. Distribution of activity at
rest is normal.
There are no fixed defects and no
areas of ischemia. The walls
brighten normally during systole.
IMPRESSION:
1. No evidence of scar or ischemia.

218312 Estimated GFR > 60 mL/min/1.73 sq
m if African American
GFR is estimated using the MDRD equation
recommended by the
National
Kidney Disease Education Program.
Estimated GFR Interpretive Information:
<60 mL/min/1.73 sq m Chronic Kidney
Disease
<15 mL/mon/1.73 sq m Kidney Failure
Estimated GFR greater than 60mL/min/1.73
is of limited clinical
Value.
The MDRD equation is not valid in the
following situations:
- Patients under 18 years of age
- Severe malnutrition or obesity
- Vegetarian diet
- Rapidly changing kidney function
Sample hemolyzed. Results for K, Total
Bili., Direct Bili., AST, LD,
