The osseous structures are intact with no fracture or focal
destruction. The joint spaces are maintained. No erosions are
identified. Small spurs are seen at the first carpometacarpal joints
bilaterally. No soft tissue abnormality is identified.

IMPRESSION:

Minimal spurring at the first carpometacarpal joints, otherwise
normal bilateral hands.

CLINICAL HISTORY: Hyperthyroidism secondary to Graves Disease
PROCEDURE: After a PARQ conference was held with the patient
(including explanation of the risks, benefits, and alternatives), the
patient consented to receive radioactive iodine treatment for
medically refractive hyperthyroidism. The patient's recent I-131
uptake was 17.5 percent. A dose of 23 milliCuries was calculated and
agreed upon for the patient by consultation with the referring
physician Dr. Vanek. After positively identifying the patient, the
patient was given an oral capsule containing 23.2 milliCuries of
I-131. The patient was sent home with printed post therapy
instructions to prevent radiation contamination. No immediate
complications occurred.
IMPRESSION:
Successful administration of 23.2 mCi I-131 for the treatment of
hyperthyroidism.

Therapy dose is calculated at approximately 23 mCi.
IMPRESSION:

Technique: MRI of the brain with and without gadolinium performed
on the 1.0 Tesla scanner with the following sequences:
Axial T1, T2, FLAIR, DWI and proton density,
Sagittal FLAIR,
Sagittal T1 weighted,
and Post-contrast axial and coronal T1 weighted images.
20 cc of intravenous Omniscan was administered for this study.
Comparison: MRI from 3/20/06.
History: Numbness and tingling.
Findings: The cisterns, ventricles and sulci appear normal
with no evidence of hydrocephalus or mass. The flow voids of the
cerebral vasculature appear normal. There are no areas of
diffusion restriction is suggest infarction. No intraparenchymal
hemorrhage or extra fluid collections are seen. There is no mass
effect or shift of midline structures. There is no abnormal
enhancement on the post gadolinium studies. Mucosal thickening is
present within the left maxillary sinus, with near complete
opacification.
Impression: Unremarkable MRI of the brain with and without
gadolinium.

COMPARISON STUDIES: None

HISTORY: Chest pain.

TECHNIQUE: The stress portion of the study is first performed
under the supervision of nursing. EKG, BP, and other pertinent
details are recorded in the chart. The patient is infused with
59.6 mg dipyridamole; 32 mCi sestamibi is injected two minutes
after the completion of the dipyridamole infusion. Gated SPECT is
acquired one hour after the conclusion of the infusion.

The rest portion was performed the following day, the patient is
injected with 10.2 mCi Tc 99m sestamibi. Gated SPECT imaging is
acquired one hour after injection.
SPECT and gated images were evaluated.

FINDINGS: A small defect at 11 oclock is thought to be RV
insertion. Distribution of activity on stress images is otherwise
normal. Distribution of activity at rest is normal.
There are no fixed defects and no areas of ischemia. The walls
brighten normally during systole.
IMPRESSION:
1. No evidence of scar or ischemia.
